Job Requirements of Clinical Educator - Post-Acute Care:

- 2-3 years clinical and technology training experience

- Excellent communication and people skills; oral, written, and presentational
- Professionalism, strong leadership skills; excellent facilitator/mentor, excellent interpersonal skills, and the ability to work collaboratively across departments
- Out-of-state travel required. Must possess valid State Driver’s License
***Administrative Contract Hours: The Clinical Educator (CE) role is a 100% a Full-time Administrative role. The Clinical Educator is not permitted to work in any clinical capacity providing patient care Monday through Friday from 8am-5pm, during their 8-hour administrative work hour days.
***Allowable FFS Work Hours: The CE is permitted to work clinically after their 8-hour administrative workday is completed or on weekends for the sole purpose of achieving their required clinical hours as required by their state license and board certification of 1000 hours every five years.
Compensation is an estimated salary range of $110,000 to $140,545 annual + a competitive benefits package.

This position will work remote when not traveling @ 30% to facilities located in Upstate New York. Candidates must live within the geography for easy travel.
Travel Requirements: Travel will be required to CE respective markets when there are identified opportunities for improved performance for a new hire, or existing clinician as identified and requested by the Regional Medical Director (RMD) or Vice President of Operations (VPO).
OVERVIEW:
The Clinical Educator (CE) prepares all clinicians to work in the Post-Acute Care environment (PAC) educates and nurtures a Value Based Practice culture and assists the leadership team in assessing individual and group productivity/efficiency.
The Clinical Educator will possess a strong understanding of PAC environment best practices that lead to quality of care, patient safety, and the effectiveness of facility interpersonal relationships, and enhanced clinician productivity and performance and be able to translate this understanding during education.
RESPONSIBILITIES:
- Conduct new hire orientations in compliance with Zenith PAC Quality Integration Channel formal process platform
- New hire clinician performance during the first 70 days of their new hire journey to include: How to navigate GEHRIMED, use the GEHRIMED Scheduler, census clean up and discharge of patients, timely note closure, good case mix, making sure SNF POS 31 have follow up medically necessary visits scheduled, using the scheduler, proper use of ACP TAB, understand and use the CIW, importance of chart note completion within 48hrs, keeping an updated Medication and Chronic Condition list
- New Hire Journey GEHRIMED Scheduler Utilization Initiative, all newly employed clinicians are expected to use the Scheduler > 90% of the time within 21 days of starting
- Educate existing clinicians needing improvement in areas identified by clinical leadership
- Collaborate with Clinical Documentation Educator (CDE) during the orientation process, and when deficiencies are identified to ensure proper coding and documentation requirements are being met
- Support and foster a positive attitude toward new and existing PAC environment initiatives, promote and provide support and encouragement to leverage clinician performance
- Collaborate with Regional Medical Director (RMD) and Clinical Market Leader (CML) respective territories to provide clinicians with clinical education, support, and oversight
- Be a positive steward of all Value Based Care Initiatives that require clinical educator engagement, education, and tracking
- MIPS eligible clinician, education, tracking and support to meet quality performance targets, ensure clinician understanding of MACRA/MIPS, BAA, Quality Measures & selection, use of MIPS Portal and Improvement activities
- ACO, support and educate on all roadmap rollouts and required visits, quality metric target clinician education, tracking and support to achieve targets for HTN, DM/A1c, Depression Screenings, Annual Wellness Visits, Advanced Care Planning, and any other added quality measures or initiatives as directed by the National Director of Education or Group President
- Advanced Care Planning education on proper documentation and coding requirements and on how to conduct the conversation
- Hospice, LOS > 6 Months Initiative, develop an ongoing cadence to following the Hospice tracker for CE respective markets. Educate clinicians on understanding proper placement & Hospice Initiative compliance and documentation in patient chart
- Responsible for educating and working with clinicians on understanding Annual Wellness visits compliance and tracking performance
- Work with VPO and Clinical Practice Manager when there are clinical educationalneeds identified
- New Hire Orientations:
New hire orientations and coverage takes precedence before any administrative duties, travel to markets, clinical work, voluntary commitments, and meetings unless approved by the National Director of Education or PAC Group President
- New Hire Orientation Coverage:
Clinical Educators will work collaboratively to ensure that any orientations needing coverage will be covered amongst each other with an equitable distribution, adopting a proactive approach to ensure an optimal clinician experience.
