Senior Practice Support Coordinator

JOB OVERVIEW
The Practice Support Coordinator/ALF (SPSC/ALF) will have a noticeable presence in supporting ALF clinicians and coordinating patient-related documents and miscellaneous duties as assigned. The PSC/ALF will collaborate with ALF Clinicians, the Practice Support Manager, the Vice President of Operations (VPO), and the Clinical Practice Managers (CPM) to ensure the completion and efficiency of the program and maintain customer confidence by keeping information confidential.
ESSENTIAL FUNCTIONS AS ASSIGNED
- Support the ALF Clinicians
- Coordinate patient-related paperwork
- Schedule patient visits in EHR
- Fax medication refills
- Process medical record requests
- Send referrals to specialists
- EHR uploads
- Verify benefits and eligibility
- Obtain authorizations for services as needed
- Miscellaneous duties as assigned
